Our Origins are Rooted in Success
Our path to Conversion Healthcare started at the fourth largest academic medical center in the country as part of a $15 million grant from the Centers for Medicare & Medicaid Services (CMS) Innovation Center, also known as “CMMI,” to improve care for cancer patients. The non-clinical care guidance services delivered as part of this program resulted in $57 million in savings realized across five states within 36 months.
Since our inception, we have broadened the scope of the problems we solve to include sub-optimal patient acquisition, patient leakage, chronic condition management, avoidable utilization, avoidable deterioration, avoidable readmissions, CMS bundles, patient satisfaction, and remote patient monitoring. Our client base represents large, medium, and small hospital programs and medical practices of varying sizes. We also maintain a significant client base across payors and medical device companies focused on increasing awareness and adoption of their products and services.
